2012 provisional classification criteria for polymyalgia rheumatica: a European League Against Rheumatism/American College of Rheumatology collaborative initiative

Absolute requirement

Patient age ≥50 years and has bilateral shoulder aching and abnormal CRP and/or ESR

Criteria with ultrasound

Morning stiffness duration >45 minutes

Hip pain or limited range of motion

Absence of RF or ACPA

Absence of other joint involvement

At least one shoulder with subdeltoid bursitis and/or biceps tenosynovitis and/or glenohumeral synovitis (either posterior or axillary) and at least one hip with synovitis and/or trochanteric bursitis

  • These classification criteria should be applied to classify the patient as having giant cell arteritis when a diagnosis of medium-vessel or large- vessel vasculitis has been made
  • Alternate diagnoses mimicking vasculitis should be excluded prior to applying the criteria
